Telemedicine App Development: What It Actually Takes

It's More Than Just Video Chat

Everyone thinks they can build a telehealth startup by dropping a Twilio Video SDK into a React app. They are wrong. The video feed is the easy part. The hard part is the regulatory nightmare surrounding it.

What you actually have to build

  • Bulletproof Video: Don't try to manage your own TURN/STUN servers. Use a managed provider. Edge cases with mobile networks will destroy your custom implementation.
  • Real End-to-End Encryption: Patients will send sensitive images. If your database gets dumped, those images cannot be readable. E2EE isn't optional.
  • EHR Integration: If doctors have to manually copy notes from your app into Epic, they won't use your app.

You aren't paying for the UI. You are paying for the audit trails, the uptime guarantees, and the security architecture.
